INTRODUCTION: Τhe need for immediate treatment, as well as the procedure
limitations in the biopsy, forces the physicians to take decisions based on the
cytology.
METHODS: The aim of our retrospective analysis was to investigate whether it
is adequate or not to establish diagnosis, for lung cancer patients in our
institution, through cytology test only. All the data were analyzed in SPSS 17.0
RESULTS: 557 patients diagnosed with lung cancer in Sotiria Athens Hospital, by
cytopathology or histopathology until December of 2010, were analyzed. 76.2% of
them were diagnosed with NSCLC, 22.2% with SCLC and 1.6% with NOS. 80,6% of
patients were male and 19,4% were female. Only 9% were non smokers, and for
those who smoked, the average smoking habit was 69.8 packyears. The mean age
was 60.9 years. 55.6% of the patients were diagnosed by cytopathology, whereas
44.6% only by histopathology. 13.5% of patients were treated with only one of
the standard regiments as 86.5% was treated with combination therapy (two or
more regiments). 29.8% of the NSCLC patients had localized disease (IA-IIIA)
whereas 70.2 % was staged as advanced disease (IIIB and IV). Most of the
patients were in a good PS at the time of diagnosis (85.6%). Log-rank test
(Mantel-Cox), for TTP, revealed no significant difference (p=0,276) for the
different levels of pathology test.
CONCLUSION: Our analysis shows that the decision taken by physicians to proceed
with the treatment in lung cancer patients, when the histopathology test is
impossible, is quite safe and it should be taken when there is urgent need for
the start of treatment.
