PERMANENT ANXIETY AND DEPRESSION IN PATIENTS WITH ACUTE CORONARY SUNDROMES

ABSTRACT



Introduction: Acute coronary syndromes form ST elevation myocardial infarction,
non ST elevation myocardial infarction, unstable angina and sudden cardiac
death, are major causes of hospital admissions and the main types of
cardiovascular diseases that are the leading death cause in Western countries.

Purpose: The purpose of this study is to investigate the link between anxiety
and depression in hospitalised patients suffering from Acute Coronary Syndromes.

Material and methods: The population of the study included 195 patients who
suffer from ACS and hospitalised in G.H “Konstantopoulio” in New Ionia, Athens
from September 2013 until April 2014. For data collection has been used the
PHQ-9 questionnaire (Patient Health Questionaire-9) of Robert L. Spitzer,Janet
B.W.Williams, Kurt Kroenke and also the HADS scale (Hospital Anxiety and
Depression Scale) of Zigmond and Snaith. For the analysis of the data was used
the statistical package SPSS-15 and the method t-test and Anova

Results: Of the 195 patients of the group the 64.1% were males and the rest
35.9% were females. According to the analysis, was found that women experience
higher depression levels p<0,035. Regarding different ages, lower depression
levels has the group over 54 years old. On the other hand higher depression and
anxiety levels appear in age group over 75 years.

About marital status found that higher levels of depression experienced by
unmarried, widowers and those who live separated or divorced p<0.004.

About different professional status higher depression levels experienced by
freelancers p<0,003, unemployed and householders have higher anxiety levels,
p<0,001 & p<0,002. Regarding the place of residence was found that those who
live in the countryside experiencing greater stress intensity p<0.0028.

About medication, found that people who take heart-antidiabetic drugs have
higher depression levels. About the number of the children, people who have
more that 2 children experiencing higher depression levels p<0.01
People who have been hospitalised in the past for the same reason, have higher
rates of depression , and also experience higher stress levels than patients
suffering from another serious illness . Regarding the degree of awareness ,
higher levels of depression and anxiety experienced by people who were not very
well informed and higher levels of depression experienced by people who live
alone or with an employed person.

There were no statistically significant correlation regarding education level ,
class of coronaty disease the preexisting cardiac disease and family history .

Conclusions: The conclusion of this research is that clinical and
sociodemographic factors are related to the intensity and the occurrence of
anxiety and depression in patients with acute coronary syndromes.
