Summary:
The anabolic activity of chondrocytes in normal mature cartilage consists
principally in continuous production of aggrecan and proteoglycans, in order to
maintain homeostasis of its matrix, while the needs for collagen synthesis are
almost zero.
In fetal cartilage, during growth, in osteoarthritis (OA), and
osteophytes, chondrocytes are requested to construct a new tissue, resulting
in, the diversion of their synthetic activity to the production of collagen,
against proteoglycans. These phenotypicaly modified chondrocytes produce
enzymes, cytokines and growth factors that irregulate the normal cartilage
turnover and promote its matrix degradation.
The metabolic biomarkers of articular cartilage are divided into:
1. Metabolic markers of Type II Collagen (CII),
2. Metabolic markers of Aggrecan,
3. Metabolic markers of other, non-colleagenous molecules,
4. Markers of Metalloproteinases (ΜΜΡs) and their Tissue inhibitors
(ΤΙΜΡs).
These metabolic markers of the articular cartilage are determined only by the
ELISA technique (Enzyme Linked Immunosorbent Assay), because they have not yet
been applied in automatic immune analyzers, on behalf of the diagnostic
companies.
Also, new genetic, proteomic and metabolic approaches have revealed many
information for new markers that contribute to the diagnosis and medical
treatment of a joint disease.
