Summary:
Aortoenteric fistulas are a rare cause of gastrointestinal bleeding. The high
mortality
associated with this clinical condition and the relatively low incidence makes
this a
diagnostic and management challenge. Given the older age and comorbitities of
these patients, conventional open surgical repair of the fistula can entail high
morbidity and mortality in the setting of a potentially hostile abdomen and
significant blood loss. An endovascular repair offers significant advantages
for such
patients. However there are potential problems with endovascular repair. The
previous graft, which is colonized by intestinal flora, is not removed and the
fistulous
track to the aorta is not physically disrupted.
In the present review to the litrature we attempt to compare the endovascular
with
the open aortoenteric reconstruction method and their short-term and the
longterm
results.
The endovascular method was shown to have fewer complications and lower
mortality, but this short term advantages are lost later in the follow up
mainly due to
recurrent sepsis.
Therefore endovascular repair of aortoenteric fistula is indicated as a
bridging option
in selected patients.
