Summary:
BACKGROUND: The post-cardiac arrest syndrome is a medical field available for
clinical trials of treatments, which could potentially be utilized to improve
outcomes of patients resuscitated from cardiac arrest. However, there are gaps
in knowledge waiting to be filled.
OBJECTIVES: To conduct a systematic review of therapeutic interventions of
arrhythmias following the Restoration Of Spontaneous Circulation (ROSC).
SEARCH METHODS: We conducted a rigorous research of Pubmed. We identified
letters, experimental, prospective or retrospective cohort, observational
case-control studies referring post ROSC interventions in adults utilizing
combinations of keywords such as: in-hospital or out-of-hospital cardiac or
heart arrest, after cardiopulmonary resuscitation, post-cardiac arrest syndrome
or arrest or period or phase, post-cardiac arrest care or cure or treatment or
therapy.

SELECTION CRITERIA: We searched all available studies in humans or animals from
2012 to 2015 that could potentially solve the problem of dysrhythmias after
successful resuscitation. Reviews, articles not written in English and
pre-arrest or during arrest interventions were excluded.
DATA COLLECTION AND ANALYSIS: Two authors independently assessed trial quality
and extracted data.
MAIN RESULTS: The search yielded 102 potential studies for further review.
Thirty four studies met the inclusion criteria, of which 13 were
experimental/randomized controlled trials, 7 prospective cohort studies, 12
retrospective cohort studies, 1 observational case-control study and 1 letter.
Our main finding is the heterogeneity in the selection and reporting of
outcomes that limited safe data and therefore guidelines for each therapy.
AUTHORS’ CONCLUSIONS: Currently there is inadequate evidence in available
therapies. There is need for further clinical trials on the subject.
