Summary:
Pregnancy is a condition in which changes occur in the hemostatic mechanism of
the body, which creates a hypercoagulable state. This helps to maintain the
function of the placenta and in the reduction of blood loss during delivery,
but can cause thrombosis and severe complications during pregnancy and
postpartum.
Thromboelastography is a point of care monitor of whole-blood coagulation
expressing in parameters all the basic functions of hemostasis – primary
hemostasis, secondary hemostasis and fibrinolysis.
The purpose of this thesis is to define the normal range of
thromboelastographic parameters in different trimesters of pregnancy, during
delivery and postpartum.
Blood samples from a random population of women were studied in 4ml vials with
the addition of citrate acid with the use of TEG during the first, second and
third trimesters of pregnancy and during childbirth, delivery by caesarean
section and the second day after delivery .
Using this criteria, the study was carried out on 24 women in the first
trimester, 26 women in the second trimester, 36 women in the third trimester,
22 during delivery by cesarean section, 9 during vaginal delivery and 8 women
in puerperium.
Results were recorded for 4 different parameters: R (clotting time), K
(clotting time), a Corner, MA (maximum clot strength).
The data from our study confirms the data from literature and clinical
experience according to which pregnancy and especially the puerperium are
periods of high risk for thrombosis and objectively thromboelastography may
indicate that this period of risk remains high throughout pregnancy, delivery
and puerperium.
