Summary:
Endovascular repair of thoracic aorta, also referred to as (TEVAR), refers to
a minimally invasive approach that involves placing of a graft in thoracic
aorta or thoracoabdominal aorta for the treatment of various pathologies of
the thoracic aorta. The purpose of this study was to review the major health
care outcomes in patients undergoing thoracic Endovascular aneurysm repair
(Endovascular Thoracic Aneurysm Repair – TEVAR). The significant lack of
relevant studies from the Greek, primarily literature was the main trigger for
the preparation of this study. The electronic databases Medline, Web of
Science, Scopus and Embase were reviewed during March 2016. In the end, only
17 studies met the pre-defined criteria in this review and formed the material
of our study. As to the frequency of observed complications, the emergence of
endoleak ranged from 4 to 33.3%, ruptured aneurysm from 0 up to 13.3%,
retrograde dissection from 0 to 6% and the growth of the aneurysm distally or
even in the false lumen from 0 to 21%. Finally, redo surgery was needed in
percentages from 0 to 80%. According to the results of our study, the various
studies that were reviewed, the survival of patients ranged in > rates 70% even
in 5 years after the initial surgery. Complete thrombosis of the false lumen
after TEVAR was observed in the majority of studies which reviewed and
investigated the specific outcome in more than 85% of patients. From the
literature review do not seems to indicate strong evidence about the
important of superiority and the emergence of more favorable medium-and
long-term outcomes in patients. Further research is needed through multi-centre
research and perspective research design, which will include bigger sample
size in order safer conclusions about the contribution of TEVAR in
improving the medium-long term outcome of the patients.
