Summary:
Acute type B aortic dissection is a catastrophic condition of the aorta and
comprises approximately 1/3 of acute aortic syndromes. Despite its low
incidence in general population, the rates are quite greater in older
population. The long-term prognosis of this pathology is poor and the
scientific community focuses in improving its therapeutic management. Nowadays,
therapeutic options for type B aortic dissections are best medical therapy,
open surgical or endovascular treatment, or their combination. Mortality and
morbidity rates after an open surgical approach is 35% approximately, getting
up to 50% for type IIIb dissections. Endovascular repair (TEVAR) has been shown
to surpass and replace to a certain extent the open surgery, though mortality
and morbidity rates of the first are not negligible. International guidelines
recommend best medical therapy for uncomplicated type B dissections and
endovascular repair for the complicated ones. Recent data indicate that a great
amount of patients with uncomplicated dissection will not survive in the
long-term due to this pathology or will need to be operated on due to
complications in the near or distant future. Thus, risk factors have been
determined to predict and define those patients who could benefit from early
endovascular repair, in order to prevent future lethal complications. Studies
still carry on to determine the risk stratification and prevent from
unnecessary endovascular interventions. We present herein literature review on
this topic, which shows the importance of endovascular techniques in improving
the long-term outcome of type B aortic dissections. The role of endovascular
treatment in uncomplicated type B aortic dissections is expected to be further
studied, in order to define accurately the proper strategies of intervention.
Patients presenting with acute type B aortic dissection should be treated in a
personalized basis considering the peculiarity of this catastrophic pathology
and the uniqueness of every patient.
