Summary:
General medical settings have been called the “de facto mental health care
system”. The majority of older adults with depression is being treated in busy
primary care settings where staff and time are limited. Routine systematic
screening instruments in primary care has been endorsed by international
professional organizations , though depression in older adults remains
unrecognized and under-treatment in clinical practice. Selection of a screening
instrument , is the first step in the process of detecting geriatric depression
in primary care settings . Systematic screening instruments may be helpful
for physicians to indentify the disorder in older adults earlier. Geriatric
depression in primary care settings is seriously undetected , undiagnosed and
undertreated. Several tools are available to facilitate screening for
depression in older adults, but few have been validated in geriatric
populations.
The aim of present study is to review screening instruments to identify
depressive symptoms in older adults earlier, in primary care settings by
presenting the most commonly accepted screening instruments of depressive
disorders in older adults.
The results are :the self-report instruments as GDS-15 , PHQ-9 , CES-D 10 ,
PHQ-2, BDI-PC, have adequate psychometric properties for use as screening
instruments for depression in older primary care patients.
A two-stage screening process is usefull when using extremely brief first-level
screens followed by more intense second –level longer screening tools .Such
brief screening tools can be easily administered by office staff or physicians
during a primary care visit.
