Summary:
Aim
The objective of this clinical trial was to evaluate if patients with severely
resorbed mandibles can benefit from immediate dental implant therapy and also
verify whether they have less denture complaints, feel more satisfied and rate
their chewing comfort and ability higher after treatment. Treatment efficacy
and satisfaction was evaluated via a questionnaire and by documenting
prosthetic complications in conjunction with implant and prosthesis survival.
The overall aim was to assess the feasibility, safety and potential of this new
treatment modality while simplifying therapy by avoiding bone grafting
procedures. It was anticipated that the study would yield data essential for
the evaluation of a treatment plan, focusing on the expectations of the patient
and not only on the technical characteristics of the prosthetic restoration.

Materials and methods
In 2007, a prospective clinical trial was set up in the University of Athens in
order to evaluate the efficacy of immediate loading of dental implants. Ten
subjects with extensively atrophic arches were examined and participated in the
study. Initially, all subjects received a new set/pair of conventional
dentures. After a thorough pre-surgical evaluation, four Ankylos implants were
placed in the edentulous mandible of each patient, connected with prefabricated
telescopic abutments, and splinted with an overdenture immediately after
surgery (immediate functional loading). At 3 time intervals we evaluated on
basis of a questionnaire the patient’s satisfaction from their lower
overdentures regarding function, esthetics and speech along with the
cost-effectiveness of the protocol. Also, all prosthetic complications were
recorded.

Results
After one-year, all subjects with mandibular implant-retained overdentures
reported fewer functional problems and denture complaints. For the total of 40
implants a survival rate of 100% has been observed. All subjects with
mandibular implant-supported overdentures were completely satisfied from the
immediate loading procedure and reported that their prosthesis stability and
chewing efficiency was significantly higher, thus influencing their dietary
intake. Also, ease of hygiene and wearing comfort were reported. After 2-4
years of function only a few and mostly minor complications were observed.

Conclusions
The results of our study confirm that immediate loading procedures with the
SynCone-system is a satisfactory treatment for subjects having retention and
stability problems with their mandibular dentures due to dramatic alveolar
ridge resorption. Furthermore, it seems that pre-fabricated double crown
attachments provide a viable alternative treatment option concerning a
simplified clinical procedure, financial advantages and high predictability.
At this moment, almost 4 years after treatment, it can be concluded that the
advanced long-term mechanical stability of the telescopic implant-supported
restorations allows for reduced incidence of complications and relative low
maintenance cost.
