Summary:
To assess the effect of different levels of intervention on halitosis and
volatile sulphur compounds (VSCs) concentrations of periodontitis patients
before and after non-surgical periodontal therapy.
MATERIAL & METHODS:
This clinical trial included 11 adults with advanced chronic periodontitis and
halitosis, from the postgraduate clinic. After an initial control period,
following the initial exam, all patients received oral hygiene (OH)
instructions and tongue cleaner. Non-surgical periodontal therapy was
completed within 48 hours after the one week of OH and tongue cleaning.
Examinations were at baseline, 4 weeks without any intervention, 1 week after
OH and tongue cleaning, 6 weeks post non-surgical therapy. This included Winkel
Tongue Coating Index (WTCI), Organoleptic Score (OLS) of nose and mouth air,
VSCs measurement by the Oral Chroma™, plaque index (sPlI), gingival index
(GI-s), bleeding on proding (BoP) and pocket depth (PD).
RESULTS:
No difference was determined in any of the parameters examined during the first
4 weeks of control. sPlI, GI-s, BoP, WTCI, OLS of the mouth air and hydrogen
sufide concentration showed statistically significant differences (p<0,05)
after one week of OH and tongue cleaning. A further significant decrease, below
the level of displeasure, was determined 6 weeks after non-surgical therapy for
sPlI, GI-s, BoP, WTCI, PD, OLS of mouth air and methyl-mercaptan concentration
(p<0,05).
CONCLUSION:
Effective OH and tongue cleaning improve the OLS of mouth air and reduce the
concentration of VSCs. In patients with advanced periodontitis, periodontal
therapy further improves the OLS of mouth air and reduces the concentration of
VSCs.
