Summary:
The aim of the present thesis was to determine plasma levels of colistin in
critically ill patients after intravenous administration of the prodrug
Colomethylsulfate- CMS that is hydrolyzed in vivo to the active drug colistin.
High pressure liquid chromatography with fluorescence detection (HPLC-Fluoresce
detection) was used for colistin assay in plasma of critically ill patients. A
pharmacokinetic analysis of these data showed that the calculated values of the
drug concentration at steady state (loading dose of 9 MIU and maintenance dose
of 4.5 MIU every 12 h) achieved the clinical target according to EMA and FDA
guidelines and in addition all patients have cured (except one). Clearance of
colistin appears to be linearly correlated to creatinine clearance in contrast
to other pharmacokinetic parameters which seem to have no correlation. The
study is ongoing with the addition of a larger number of patients.
