INTRODUCTION: The limited availability of blood requires the rational use, in order to avoid unnecessary transfusions and their complications. The main purpose of this study was to identify and analyze the number of transfusions in cesareans sections (CS), to improve the efficiency of ordering and blood management.
MATERIALS AND METHODS: We studied 1762 CS performed 2013-2015 in Obstetrics Clinic "Aretaieio". They analyzed the following data: age, number of units of red blood cells (RBCs) cross-matched, and strictly provided on the day of surgery. Calculated the following factors: a) C / T ratio (No. of units cross-matced / No. of units transfused) a ratio > 2 consider indicative of significant blood wastage b) Transfusion propability × 100 (No. of patients transfused * 100 / No. of patients cross-matched) a value of <30 was considered indicative of significant blood wastage and c) Transfusion index (No. of units transfused / No. of patients cross-matched ) a value of <0.5 signifies no need for cross-matched. Meanwhile, it’s been estimated the total cost of each intersection (operating and consumables).
RESULTS: Of the 1,762 KT, cross-matched RBCs for 321 CS (18,21%), and finally transfused 37 women (11.54%). From 688 RBCs cross-matched, requested 103 RBCs (14.97%) averaged 2.8 RBCs / woman (Table 8). All C / T ratios 6.68, probability transfusion 11.53, transfusion index 0.32, is indicative of the absence of real need for cross-matched RBCs in CS.
CONCLUSION: In our study highlighted the excessive demand and unnecessary cross-matching RBCs in CS. The determination of blood group without advance intersection RBCs in pregnant women appear to be adequate in most cases. The fixing of specific prognostic transfusion characteristics (age, obturator placenta), would contribute to better management of blood stocks in reducing complications from unnecessary transfusions, while lowering the cost of unnecessary crossings.
