Many studies have shown that individuals with an autoimmune disease are at increased risk of developing another autoimmune disease. Psoriasis is a common chronic inflammatory disease of the skin, affecting 1-3% of the population in Europe and North America. Sometimes patients with psoriasis develop psoriatic arthritis (10-15%). Psoriatic patients have an increased risk of developing a second or third autoimmune disease. Autoimmune thyroiditis is an autoimmune disease of the thyroid and is supposed to be associated with psoriasis. This is the reason why their relationship has been studied, with contradictory results so far.
The main purpose of this study was to investigate whether patients with psoriasis (with or without psoriatic arthritis) have an increased risk of developing autoimmune thyroiditis, compared with controls (without psoriasis or known thyroid disease). Secondary, the study aimed to compare the characteristics of psoriasis between patients with autoimmune thyroiditis and patients without autoimmune thyroiditis. Finally, a subgroup of the patients was taken that consisted of patients with psoriatic arthritis. This subgroup was compared with the controls in relation to the presence of autoimmune thyroiditis.
The type of this study is case- control. Patients with chronic plaque psoriasis were recruited from the Dermatology Department of Attikon University Hospital. Controls without psoriasis or known thyroid disease were selected from the outpatient Endocrine and Obesity Clinic. For each patient there were selected two controls of similar age and body mass index (bmi). All participants underwent a complete medical examination and gave informed written consent.
Descriptive statistics was used to analyze the clinical and laboratory characteristics of cases and controls. Statistical tests were used to compare the characteristics between the two groups and logistic regression models were used in order to investigate the aims of the study.
As far as the results are concerned, no association between psoriasis and autoimmune thyroiditis was detected. Also, there was no difference at the characteristics of psoriasis between patients with autoimmune thyroiditis and patients without autoimmune thyroiditis. In addition, no association between psoriasis and autoimmune thyroiditis was found when the subgroup of patients with psoriatic arthritis was compared to the controls. One important finding of this study was that gender seemed to have a statistically significant impact at the presence of autoimmune thyroiditis at the control group. This relationship was not significant at the patients group.
Limitations of the study are restriction of the findings at the geographical region of Greece, relatively small sample size of patients and especially those with psoriatic arthritis and, finally, the inclusion of psoriatic patients seeking medical care in a tertiary centre probably caused underrepresentation of mild forms of psoriasis. Strengths of the study are the selection of participants living in an iodine-replete area, well-characterized patients and controls and homogeneity concerning ethnicity of the participants. Further studies with larger samples would be useful to be conducted in the future.
