Summary:
Early application of enteral nutrition program in patients with acute pancreatitis has begun to gain ground in recent years, putting into question the usefulness of total parenteral nutrition. The avoidance of pancreatic activity is not related to the reduction of morbidity or mortality. On the other hand, it is responsible for the atrophy of the intestinal mucosa and interruption of the protective intestinal barrier, resulting in increased complications due to generalized infection of human organism.
AIM: To investigate the effectiveness of artificial nutrition, the importance of early enteral feeding and the role of immunonutrition in the stem of complications and improvement of patients with severe acute pancreatitis.
MATERIAL AND METHOD: Systematic review on electronic databases Scopus, Pubmed, The Cochrane Library and Scopemed. The search involved only prospective studies and randomized clinical trials. Outcome measures were the reduction of systemic inflammatory response, the length of hospitalization, the rate of complications and mortality.
RESULTS: Three prospective studies and eleven randomized clinical trials were identified including 1244 patients. The superiority of enteral versus parenteral nutrition is confirmed in 6 clinical trials while early enteral nutrition is associated with lower rates of complications and mortality. The length of hospitalization was shorter for patients undergoing enteral nutrition and even less for patients receiving immunonutrition as well. In a prospective study, there was no difference either in rates of complications or in length of hospitalization while 3 studies found evidence of superiority of early enteral nutrition on elimination of mortality.
CONCLUSION: The administration of early enteral nutrition with or without immunonutrition contributes decisively to reverse the deterioration of patients with severe acute pancreatitis and it is associated with fewer complications compared with total parenteral nutrition.
