Summary:
Introduction: Failure of adherencetotreatmentproposed by the physician
topatients who suffer fromatrialfibrillation andheart failure is a matter of
concern to researchers in recent decades. The result of non-adherence is the
deterioration of health condition and the increase in the cost of care.
Aim:of the current research was to assess the degree of adherenceto
treatment and diet of patients with atrial fibrillation andpatients with heart
failure as well as the investigation offactors that affect the above adherence.
Method: Two research tools were created based on the Morisky Medication
Adherence Scale (MMAS-8) and the questionnaire by Da Silva et al. The
sample of the research was consisted of 200 patients (100 with heart failure
and 100 with atrial fibrillation) who were hospitalized in General Hospital of
Athens "Attikon" and in the cardiology clinic of "Health Melathron". The data
were analyzed with the SPSS 17 statistical program.
Results: 69% of patients withatrial fibrillation
adhereadequatelytoanticoagulation regimenrespectively57% of patients with
heart failure adheretotheirmedicationregimen. 96.7% of the 61 patients
receiving asenocoumarol complied with the regular control of INR and as a
factor of adherence the patient-doctor communication was found (p=0,045).
55.7% of patientswithatrialfibrillationand 90% of patientswithheartfailuredo not
have sufficient adherence totheirdiet and habits.
Adherencetomedicationregimenof patientswithheartfailureisaffectedbygender
(p= 0,041), educational level (p=0,014) and patient-doctor communication
(p=0,018).
Conclusions:Adherenceis a complex and multifactoralissue.The mainfound
of thisresearchis non-adherencetomedicationregimen anddiet. The
factorthoughpatient-doctorcommunicationisinvolved in highadherenceto
INRcontrol of patientswhoreceiveacenocoumarolaswellas in
adherencetomedicationregimen ofheartfailure.
