Summary:
Introduction: Under the pressure of the global financial crisis for shortcut in budgets and alignment with international recommendations, constant search for better management and control of resource allocation in the health sector, is the new reality for modern Health Systems. The Diagnostic Related Groups (DRGs) hospital payment system appears to be effective in reducing hospital costs and benefit the patient as well as the healthcare providers. In Greece, the implementation of this system is at an early stage and includes the introduction of “KEN” and its matching with International Classification of Diagnosis and cost of hospitalization.
Purpose: The purpose of this study was to describe the frequency and cost of hospitalization of orthopedic cases in a Military Hospital of Athens for the period 2012-2016.
Method: A descriptive retrospective study was conducted on a preselected population of orthopedic cases introduced at the Military Hospital of Athens for the years 2012-1016. Data gathered related to the year, sex, age group, diagnosis, days and total duration of hospitalization and its cost. For the analysis of the variables the SPSS statistical program 19 was used.
Results: The majority of the population selected was under 34 years of age, of which two-thirds were males. The median duration of hospitalization for all patients was 2 days. The main diagnoses concerned meniscal rupture, knee sprain and femoral neck fracture. There was an increase in the median total cost of “KEN” per diagnosis and per year, while the actual days of hospitalization tend to decrease year by year to converge with the days of hospitalization on the KEN basis.
Conclusions: The hospital payment through DRGs-KEN seems to promote better recording and collection of financial data, facilitate better clinical decisions and reduce the cost of patient hospitalization. There is still a need to review the days and cost of hospitalization based on DRGs-KEN.
