Summary:
Imaging investigation of intracranial atherosclerosis is currently under extensive research, as this disease has been characterised as the main cause of ischemic stroke worldwide with higher prevalence in Asian populations. Conventional intracranial atherosclerosis imaging methods have been based on imaging the arterial lumen and characterisation of luminal stenosis. The introduction of advanced Magnetic Resonance vessel wall imaging techniques (MR-VWI), has allowed imaging of vascular wall abnormalities. Studies on the characteristics of atheromatous plaque on VWI, have allowed the characterisation of unstable plaque and have provided an insight in the prediction of ischemic stroke risk. Moreover, clinical studies have successfully applied VWI to differentiate intracranial atherosclerosis from other intracranial vasculopathies, such as primary CNS vasculitis and reversible cerebral vasoconstriction syndrome (RCVS). Research on intracranial atherosclerosis imaging methods continuous to develop rapidly, utilising advanced imaging techniques that allow the combination of luminal and vessel wall imaging. The application of modern imaging methods in clinical practice is expected to direct new advances in clinical investigation and monitoring of patients with intracranial atherosclerosis and serve as measures for the assessment of new therapies.
