Summary:
Type 1 diabetes is characterized by destruction of pancreatic β - cells, leading to a severe disorder of insulin secretion as well as abnormal a - cell function leading to elevated glucagon levels in both fasting and postprandial states. Appropriate insulin therapy is central to managing all people with type 1 diabetes mellitus. However, it is deficient and many patients fail to achieve their glycemic and metabolic goals. There is also an increasing trend in overweight and obesity among people with type 1 diabetes, possibly due to insulin resistance, increased insulin dose requirements, and poor glycemic control. Consequently, attention is focused on new non - insulin therapeutic approaches that reduce hyperglycemia and improve metabolic variables without increasing the risk of hypoglycaemia or other side effects. The present study aims to review the literature on the co - administration of pramlintide, GLP - 1 receptor agonists, dipeptidyl peptidase - 4 inhibitors (DPP - 4 i), glucose - sodium co - transporters (SGLT2 i) inhibitors, metformin, sulphonylureas, thiazolidinediones and acarbose, anα-glucosidase inhibitor, in insulin therapy, to study their efficacy as well as to define the framework for future tests to clarify the characteristics of the patients with type 1 diabetes who will benefit from non - insulin treatment approaches.
