Summary:
Introduction: Non-adherence to complex therapies is an internationally recognized problem in the treatment of chronic diseases.
Objectives: The systematic review of international literature on non-adherence to the treatment of patients with chronic renal disease (CRD) under hemodialysis.
Material and Methods: International literature was searched in "PubMed" database without chronological limitations. The search was conducted from March to April 2018. The key-words used were: ‘’adherence’’, ‘’non-adherence’’, ‘’compliance’’, ‘’non-compliance’’, ‘’medication’’, ‘’hemodialysis’’, ‘’end-stage renal disease’’, ‘’chronic renal failure’’. The literature search was based on the following selection criteria: randomized trials, studies in humans, studies published in English, studies in patients >18 years old.
Results: A total of 8 studies met the search criteria. The mean rate of non-adherence to oral medication in patients with CRD under hemodialysis was found to be 50%. Oral phosphorus binding drugs was the most examined in the articles of this systematic review. The factors associated with non-adherence to the medication of these patients include age, complexity of the prescribed regimen, the large number of doses, the large size and unpleasant taste of the tablets, depression and inadequate social support. In 6 out of the 8 studies it was found that the training of these patients about their disease and medication can contribute to a significant increase in their adherence to the pharmaceutical regimen.

Conclusions: The adherence of patients with CRD under hemodialysis to their medication is considered inadequate, with many factors associated with this problem. Patient education is the solution, but more studies are required for safe conclusions.
