Predictive factors in breast cancer targeted therapy
Summary:
Breast cancer is the most frequent malignancy in women globally, and its therapeutic approach remains one of the biggest challenges of our time. In the era of personalised medicine, targeted agents against altered signaling pathways specifically expressed in the cancer cells, while sparing the normal cells, represent the new generation of anti-cancer treatment. Targeted therapy has shown the most promising results in terms of patient benefit and minimum toxicity, as its rationale is based on the identification of predictive biomarkers to guide treatment decision making.
The discovery of ER, PR and HER2 receptors and their application in clinical practise as crucial predictive biomarkers have revolutionised our way of understanding breast cancer. Since then, the evolution of genomic profiling techniques have led to further elucidating of the cancer-related signaling pathways («hallmarks of cancer»), and new therapeutic agents have been developed to target these exact pathways. However, not all tumors respond to targeted treatment. De novo and acquired resistance remains a significant obstacle, and further research to address this issue is required in order to overcome it.
