Summary:
Introduction – Background: Cancer pain is one of the most common signs of cancer that afflicts the oncology patient and needs to be treated. However, many seem to be those factors that act as barriers to the effective management of cancer pain, not allowing patient relief from both their own as well as their providers and health professionals as well as from the national health system itself.

Aim: This systematic review aims at investigating those barriers that affect the effective alleviation of cancer pain and prevent the oncology patients from relieving their symptoms.

Methodology: A review has been made in an international literature on the subject in order to best reflect the modern reality around it. More specifically, a search was made in the PubMed bibliographic database from 25/2/2018 to 25/4/2018 with key words, and a total of 188 articles were recovered, since 1990 and then, of which 27 were selected according to the criteria set.

Results: Patients and caregivers seemed to suffer and not show the pain they experienced because of their fatalities that cancer is inextricably linked to pain because of the perception that the "good patient does not complain because of the fear of tolerance, the addiction and side effects of analgesic drugs as well as the perception not to distract the attention of the treating physician. These perceptions were mainly supported by older and low-educated people. For health professionals, barriers to effective pain relief have been their unwillingness to prescribe opioid drugs, the fear of addiction and the side effects of analgesics, their lack of education and the lack of knowledge, the reluctance of nurses to provide them, and Finally, the current legislation on the prescription of a particular category of medicines
Conclusions: It seems to be clear that the barriers on all three sides are numerous and prevent the effective management of cancer pain, with the result that the oncologist is not sufficiently relieved by the symptoms. There are quite a number of things that can and should be done to improve the current situation in every direction, both by proper information, adequate education, psychological support, better communication, amending the legislative framework on prescribing opioid drugs, etc.
