Post traumatic stress in children after hospitalization in pediatric Intensive Care Units
Summary:
Introduction: More than 200,000 children and adolescents are entering pediatric intensive care units (PICU) in North America to treat health-related illnesses. What is of concern is the possibility of negative psychological consequences for children hospitalized in PICU. Children hospitalized in PICU may be at risk of developing a psychiatric illness with post-traumatic stress disorder.
Objectives: The purpose of this systematic review is to critically examine the research literature on the post-traumatic stress disorder posed by children after hospitalization in PICU.
Material and Methods: Electronic search of literature was conducted in PubMed, Cochrane, Science Direct and Iatrotek databases from January 2013 to June 31 of 2018. The total of articles resulting from the search for international literature were 82 and 15 met the selection criteria and included in the study.
Results: The most common symptoms for children were the inability to recall aspects of the event, hypersensitivity, avoidance of thought, over reactivity in the reminder of the event, annoying thoughts and images, learning difficulties, emotional difficulties, sleeping problems and the risk of having a mental disorder. The risk factors associated with the onset of the post-traumatic disorder are acute anxiety stress disorder, feeling of negative impact, fear for the hospital, chronic illness, severity of the illness, mechanical ventilation, duration of hospitalization and administration of vasoactive drugs. Also, the quality of life was lower in children with high post-traumatic stress scores.
Conclusions: Post-traumatic stress disorder that occurs in children after hospitalization in ICU is an important issue that needs to be further studied. Our review underlines the need to prevent, inform health professionals, parents and teachers, and raise awareness in order to include prevention in daily practice.
