Summary:
Introduction: Patients with intertrochanteric hip fracture are at high risk of developing pressure ulcers due to prolonged immobility before, during and after surgery and the presence of other risk factors such as poor nutritional status, increased age and diabetes mellitus.
Purpose: The purpose of this study was to investigate the incidence of pressure ulcers in patients treated with intertrochanteric hip fracture.
Methodology: This is a prospective study. The study included adult patients who were hospitalized in the orthopedic clinic of a large hospital in N. Attica with a intertrochanteric hip fracture. Demographic and clinical-laboratory data were recorded. The statistical analysis of the data was done with the statistical software SPSS for Windows (version 21), IBM statistical software (SPSS Inc., Chicago, IL).
Results: The study sample consisted of 41 patients aged 72.8±14.3 years. Most of them had normal weight (55%) and diabetes mellitus (33.3%). Pressure ulcer developed a total of 34 (82.9%) patients, while history of pressure ulcers had 19 (46.3%) patients. In 13 (38.2%) patients, pressure ulcer occurred on 5th day. 34 (82.9%) patients were ambulatory before the fracture, and 24 patients (58.5%) were discharged from hospital with ulcer pressure. Patient hospitalization was 14.8 ± 10.2 days. Patients who developed pressure ulcer during their hospitalization were significantly older (76.8±9.2 vs. 53.3±19.1, p<0.05), had a significantly lower body mass index (24.7±2.9 vs. 30±1.4, p<0.05), had a significantly higher degree of comorbidity (97.1% vs. 14.3%, p<0.05), while significantly more patients discharged from hospital with pressure ulcer (67,6% vs. 14.3%, p<0.05). The factors found to positively influence the onset of pressure ulceration were co-morbidity (OR=13,549) and diabetes mellitus (OR=12,477).
Conclusions: Patient with intertrochanteric hip fractures have high incidence of pressure ulcers. Applying appropriate prevention measures could reduce the appearance of pressure ulcers.
