The impact of psychological factors and illness perceptions on the compliance of hemodialysis patients
Summary:
Chronic renal failure affects one in eight adults and its effect is evident both in the daily routine and in the emotional state of the patient. Especially those undergoing hemodialysis treatment must re-adapt their lives by following instructions regarding their diet and medication faithfully. It has been observed that many of these patients have difficulty following the guidelines and complying with their treatment. This results in poor regulation of the disease and in the long term leads to increased mortality. Therefore, attempting to determine the factors that positively or negatively affect the compliance of hemodialysis patients with therapy can substantially contribute not only to their better care, which will be geared to their peculiarities and needs but also to the best outcome of their treatment.
This research attempts to investigate the factors that affect the compliance of hemodialysis patients. More specifically, the research focuses on demographic and clinical features, the existence of anxiety and / or depressive symptoms, and patients' perceptions of their illness. The sample consists of 65 hemodialysis patients, which had begun hemodialysis treatment at least 6 months before participating in the study. The questionnaires used are the Illness Perception Questionnaire-Revised [IPQ-R], the Hospital Anxiety and Depression Scale [HADS], and a model questionnaire for the assessment of compliance, which includes self-referencing questions in conjunction with objective evidence (patient weight, phosphorus value P).
Results showed that the majority of patients have anxiety and depression symptoms, which seem to affect their compliance with treatment. More specifically, patients with high scores on anxiety and depression scale were found more likely not to comply. It also appeared that patients' perceptions about the negative effects of the disease on their lives, the frequency of symptoms and the emotional burden resulting from the disease lead to reduced compliance with treatment. Finally, as for demographic characteristics, patients undergoing dialysis from 3 to 5 years and for more than 5 years are also more likely not to comply with treatment.
