Comparison of interventional endoscopic diagnostic methods in diagnosing lung cancer in a clinical context
Summary:
Lung cancer is the leading cause of death from malignant diseases worldwide [110], despite the discovery of new drugs and surgical treatments have progressed considerably in recent years. Interventional pulmonology, with the introduction of new bronchoscopic techniques, plays an important role in the staging and diagnosis of pulmonary disease. It helps to achieve more accurate detection rates and greatly reduces the risk of adverse events, adding to the quiver of the modern pulmonologist, minimally invasive methods for accurate diagnosis and staging of the disease. In the light of the new scientific evidence published for the early diagnosis of lung cancer [111], there is a need for safe, accurate and minimally invasive methods for the diagnosis and staging of pulmonary malignancies. In this study I present the available endoscopic technologies used to evaluate lung lesions as well as emerging techniques that could provide additional tools to complete this diagnostic approach. In addition, I present data from a 10-month (March 2017-January 2018) period of lung malignancies treated in the Oncology Clinic of "Sotiria" Thorax Hospital, where the diagnosis was made using available endoscopic techniques. A total of 200 cases were diagnosed with conventional bronchoscopy, endobronchial ultrasound or rigid bronchoscopy. The sampling was carried out using various techniques such as endobronchial biopsy, transbronchial biopsy, TBNA, brushing and bronchial washing. Type of cancer, stage of the disease, age of the patient, smoking habit, presence or not of metastases and pathological mediastinal lymph nodes and ECOG performance status were also checked, a correlation between these variables was made.
