Summary:
There is a great amount of scientific-based information and references, concerning pain and more specifically orofacial pain. In order to make the right and prompt diagnosis, it is necessary for the clinician to try and be informed at all times, so as to implement the proper treatment.
In the first part of this assignment, it is going to be described the term “Temporomandibular Joint dysfunction”. It concerns all the pathologies presented, primarily or secondary, to the temporomandibular joint system and the area around it. In the same chapter there are references as well, to the terminologies that have been used for the same purpose through the years. Basic principles about temporomandibular joint dysfunction are also cited, substantiating the data found in a large number of epidemiological researches, while emphasis is given to the dysfunction signs and symptoms, as well as to the frequency that the symptoms are presented to the population sample.
Secondly, great significance is given to the causes of temporomandibular joint dysfunction, describing thoroughly five theories, which are the condyles displacement theory, the neuromuscular, muscular, psychophysiological and psychological theories. However, a multifactor model of reasoning is supported most, minimizing at the same time, the strength of malocclusion as the main factor of dysfunction, that it was thought to be in the past. It is of great importance also, the effect of parafunctional habits, the general health problems, the systematic and psychological factors, as well as the co-morbidity along with other pathological conditions in etiology of operational temporomandibular joint dysfunctions.
In the second part of this assignment, pain classification is described in detail and pain is categorized based on two parameters: its duration and its pathophysiology. There are also mentioned the reasons causing pain, most important of which is arthritis, while there is a reference about the most frequent pain sites.
In conclusion, it is presented a wide review of orofacial pain, emphasizing as much the causes, where temporomandibular joint disorders constitute the most direct cause of temporomandibular joint dysfunction, as its classification. Moreover, data are mentioned concerning the features and pain quality, depending on the type of pain. The ultimate aim is the updating regarding the way we receive a detailed patients health history, the protocol followed during clinical examination and the need of knowledge reinforcement upon various pathologies in this specific area, so as the clinician can be guided to the right differential diagnosis and treatment.
