Summary:
A wide range of dermatological autoimmune and inflammatory diseases are a common component of the everyday workload in clinical practice. Their management is often hard to reach due to the failure of the available therapeutic options and the emergence of resistance to treatment. In view of the expanding approach of precision medicine, this study recorded the long clinical history and therapeutic strategy of ten selected adult patients with dermatological autoimmune or inflammatory diseases reported at the 'Andreas Syggros' Hospital of Cutaneous & Venereal Diseases, in Athens, Greece. These included 4 patients with psoriasis and one with pemphigus, bullous pemphigoid, subacute cutaneous lupus erythematosus, chronic discoid lupus erythematosus, localized scleroderma-morphea or dermatomyositis, receiving various immunomodulatory drugs, corticosteroids and/or biological agents based on the disease manifestations and/or occurrence of side effects. For the purpose of this study, the patients gave informed consent to collect and use skin lesion photographs and clinical data. The results showed (a) absence of a precise successful therapeutic approach for each clinical entity studied, (b) heterogeneity in treatment efficacy, and (c) frequent disease eruptions and recurrence and repeated therapeutic option modifications. The key questions that emerged and need careful consideration concern the factors that determine treatment success or failure, the link between patient and drug characteristics and the search for treatment selection markers for each patient.
