Summary:
INTRODUCTION: Pancreaticoduodenectomy is a major procedure indicated for periampulary tumor. Although perioperative mortality has been reduced in recent years, the morbidity however remains high. The major postoperative complications are pancreatic fistula, delayed gastric empting and hemorrhage. Postoperative hemorrhage is rare but may be fatal. Also may be associated with pancreatic fistula and the gastroduodenal artery stump constitutes the most common involved vessel. Several techniques have been proposed for the prevention and treatment of this complication with doubtful results. In the present study, a novel technique of “wrapping” the gastroduodenal artery stump in the jejunal serosa, so as to reduce the rate of postoperative hemorrhage, is described.
PATIENTS AND METHODS: 126 patients that underwent Whipple procedure from January 2007 to December 2017 by the same surgeon were enrolled in the current retrospective study. The suggested technique has been applied in 51 patients since January 2015.
STATISTICAL ANALYSIS: The statistical analysis has been made with statistical program STATA 13.0.
RESULTS: It seems that embedment of gastroduodenal artery stump in the jejunal serosa has no statistically positive effect in the prevention of postoperative hemorrhage.
CONCLUSION: Embedment of gastroduodenal artery stump in the jejunal serosa constitutes a novel, easy, safe, rapid and effective technique, which may prevent postoperative hemorrhage. Further research is required in order to reach a final conclusion.
