Psychotic experiences in adolescents with a history of traumatic events
Summary:
Backround: The crucial role of trauma as a vulnerability factor in schizophrenia has been widely documented in the bibliography. Currently, there has been an exponential increase in efforts aimed at elucidating what has been termed the ‘psychosis continuum' and encompasses a full range of psychotic symptom expressions fromsubclinical manifestations to the clinically significant psychotic experiences, which apart from psychosis seem to be related with other mental health issues as well (e.g., depression, suicidality, functioning impairment). In this context, examining the risk factors that contribute to the onset of psychotic experiences could be of great importance, as these factors could be the target for timely preventative interventions in mental health disorders.
Objective: The objective of this study was to examine a potential relationship between trauma and psychotic experiencesand the potential role ofpersistence and distress caused by these experiences in the psychological burdain for adolescents.
Methods: A total of 119 adolescents (12-18 years) met our inclusion criteria and participated in the study. The sample consisted of two groups: 71 participants were under institutional protection and48 adolescents came from the community. A battery of 3 questionnaires was used: 1) The Community Assessment of Psychic Experience (CAPE), a self-report tool for the screening of the psychotic experiences, 2) a self-report tool that resulted after adaptation of K-SADS-PL, for the assessment of traumatic experiences, and 3) SDQ-Hel, for the evaluation of the emotional difficulties and the functioning.
Results: The results showed a strong correlation between trauma and psychotic experiences, whilst interpersonal trauma showed the strongest correlation. There were significant positive correlations between CAPE's total frequency and distress score and total SDQ score.
Conclusion: Traumatic events are strolgly related with more frequent and distressfull psychotic experiences. Besides, distress caused by these experiences as well as their persistence proved of vital importance as a psychological burden for adolescents. Mental health professionals should take seriously into account a history of traumatic events in adolescents’ life, especially to those with frequent and distressing psychotic experiencesand vice- versa. Appropriate early interventioncould improve long-term outcomes as well as the response to future psycho-confessional conditions.
