Summary:
Breast cancer is the second most common cancer worldwide and the most common diagnosed cancer in women all over the world. The therapeutic approaches have improved over the time with new surgical techniques regarding mastectomy. However, the complications of mastectomy concern a large percentage of women, as the life expectancy has increased. Postmastectomy pain syndrome appears at 20%-55% of women who have undergone breast surgery. Its character is neuropathic and pain is located in the ipsilateral breast, the anterior thoracic wall, the axilla and the arm. It results usually from damage to the intercostobrachial nerve, the medial brachial cutaneous, the medial and lateral pectoral, the thoracodorsal and the long thoracic nerve, the neuroma pain in the region of scar, the phantom breast pain and psycho-emotional factors. Many risk factors contribute to the postmastectomy pain syndrome, which affects a lot the quality of life of the patients. The role of postoperative lymphedema is strongly highlighted. There are many therapeutic methods that are applied preemptively as well as after the appearance of the syndrome. They include pharmaceutical medication, such as antidepressants, gabapentinoids, NMDA antagonists, topical application of local anesthetics and capsaicin, as well as interventional procedures. These include wound infiltration with local anesthetics, various types of nerve blocks, ganglia blockade with the use of local anesthetics and the appliance of radiofrequency and surgical therapy with autologous fat graft. Furthermore, the role of myofascial trigger points, physiotherapy and other alternative therapies is mentioned.
