Summary:
Historically, the first reports of bile ducts were rooted during the ancient-greek period from Hippocrates and Galinos. Fifteen centuries passed since the great anatomists of Middle Age succeeded to describe in detail the anatomy above. The latest technological improvements led the opportunity to identify the anatomy of biliary tract with ten different imaging modalities.
Whilst the surgery of biliary evolves, the new general surgeon is called to face the complex situations next to high vascular structures. In almost half of the cases, the surgical anatomy of bile ducts is different from what we expected. The embryological origins of the biliary are still complicated and ambiguous, resulting in different ways of bile drainage from liver to the intestinal tract.
It is worth mentioning that preoperative approach, anatomy knowledge and constant alertness are basic rules in Bile Duct Surgery.
