Renal Vein Variations. Embryology and Clinical Significance.
Summary:
Renal vein variations are mostly clinically silent and are incidentally discovered during retroperitoneal surgeries or invasive radiological procedures. Clinical symptoms related to them are often blur, non specific and intermittent, thus leading to recurrent office visits. Awareness of the detailed anatomy of the renal veins is a major advantage prior to surgical and radiological interventions in order to avoid major complications and should always be considered in cases of chronic recurrent symptoms related to the abdomen and pelvis. The aim of the present study is to record all the anatomical variants of the renal veins, analyze and highlight their impact on medical practice and provide an up-to-date bibliographic source to all practitioners that investigate the retroperitoneal space.
A systematic review was conducted using literature search engines as Pubmed and Google Scholar, scientific web pages as Research gate, Embryology.med and LifeMap Discovery as well as the classical textbooks of anatomy and surgery. Due to limited references related to embryology and anatomical features of the renal veins, there was no limitation on the date of publication or writing. Key words were specified according to the subject of study due to the diversity of the chapter’s themes. Most references are case reports with mini literature review. Meta-analysis and systematic reviews are mainly of radiological interest. Anatomical studies and autopsy series are limited.
Additional, retroaortic and circumaortic renal veins were the most common anatomic variations that came up from the published data. Agenesia of the renal vein is reported only in 2 clinical case reports. The Nutcracker syndrome, varicocele, pelvic congestion syndrome, incidental bleeding during surgical or interventional radiologic procedures and post-surgical thrombosis were the main clinical conditions related to the anatomic variations of the renal veins.
