Catheterizing the internal jugular vein: surgical anatomy and embryology, technique, complications, and catheter tip malpositioning.
Summary:
Central venous catheters are widely used in clinical practice under different situations.
Surgical anatomy of the internal jugular vein is presented, and complications during
placement either by surgical cut-down or percutaneous cannulation are discussed on an
anatomical basis. The right internal jugular vein is preferred over the left one and is
approached in the Sedillot’s triangle. Possible complications include carotid artery
puncture, pneumothorax and malpositioning of the catheter’s tip. Malpositioning of the
catheter tip in mediastinal vessels is presented. The possible different positions of the
catheter in posteroanterior and lateral chest x-rays are explained at an anatomical basis
in normal anatomy and congenital venous anomalies.
