Introduction: Perioperative antibiotic prophylaxis (AP) is a well established factor for the prevention of the Surgical Site Infections (SSI) in pediatric cardiac surgery patients, but the adherence to the International Guidelines remains widely ranged.
Object: The aims of this study is to determine the pattern of perioperative antimicrobial prophylaxis in pediatric cardiac surgery patients and to evaluate its adherence to Guidelines for Antibiotic Prophylaxis. Furthermore, we determined the incidence of Health Care Associated Infections (HAI) and the risk of acquiring resistant pathogens.
Methods: A retrospective and prospective observational study of pediatric cardiac surgery patients who were admitted in a Pediatric Cardiac Intensive Care Unit (PCICU), during 18 months.
Results: The mean adherence rates were 66,3%, with prolonged antibiotic prophylaxis in 65,8% of patients and a major use of glycopeptides. The prevalence of HAI was 38,0%, while 29,4% of patients had a shift to resistant pathogens in their colonization. Previous colonization with ESBLs may be a risk factor for the occurrence of infections after cardiac surgery in paediatric patients.
Conclusions: The implementation of an effective prophylaxis protocol for children undergoing cardiac surgery is needed, especially in view of worldwide antibiotic overuse and increasing development of resistance. Studies are needed with randomization estimating safety and efficacy of particular schedules of antibiotic prophylaxis in children.
