Summary:
CBCT dental systems are increasingly used by radiologists and dentists for various clinical applications. Although radiation doses from CBCT are generally lower than other CT exams, CBCT generates a higher x-ray dose than conventional radiographs, so it is important that it is used when clinically indicated and with the lowest dose consistent with appropriate image quality. In the European Union, it was recognized that there is an urgent need to set standards for the use of CBCT. It was therefore decided, with the collaboration of the European Academy of Dental X-ray Radiology (EADMFR), to develop the European SEDENTEXCT program (Safety and efficacy of a new and emerging dental X-ray modality) with the aim of developing a set of "Basic Principles" based on fundamental international principles, EU directives and previous guidelines. The main purpose of this work is the determination of the selection criteria of the patients who are referred for CBCT examination to the Department of Oral Diagnosis and Radiology of School of Dentistry, National and Kapodistrian University of Athens.
Particular objectives are: a) the investigation of the distribution of clinics in the prescription of the examination b) the correlation of the justification of the examination with the clinic from which the referral was made c) the correlation of the justification of the examination with the field of view (FOV) size d) the analysis of the composition of the demographic profile of patients and e) the association of the demographic profile of patients with the justification of the examination.
For the purposes of this study, the standardized referral forms of all the examinations carried out in the CBCT unit of the School of Dentistry, National and Kapodistrian University of Athens were used for the period from June 1,2018 to December 31, 2018. This duration is considered sufficient to obtain a reliable estimate of the purposes of the study. A total of 253 referral forms were recorded during this period. During the project, there was a continuous crossing and control the data to avoid any mistakes.
The results of this study demonstrate a significant variation in the referral rates between clinics of School of Dentistry (National and Kapodistrian University of Athens) in prescribing the test. Most patients were referred from Oral and Maxillofacial Surgery and Prosthetics with rates of 33.07% and 19.29%, respectively. The main indication for referrals from of Clinical Oral and Maxillofacial Surgery Clinic is the evaluation of the position of the third molar of the mandible with the lower alveolar nerve. It was also demonstrated that the highest proportion of clinical referral criteria was for implants (34.25%). Forms of referrals at 5.12% concerned the undergraduate curriculum and 94.88% the postgraduate curriculum.
It would be interesting to repeat a similar study in private centers in adjacent areas, so to control the existence of a correlation of the profile and justification of Dental School patients with those who are seeking private dental care.
The results of this research brought the following important conclusions:
• Most patients were referred from Oral and Maxillofacial Surgery and Prosthetics Clinic.
• Most patients were referred from the postgraduate program
• The highest proportion of clinical referral criteria are implants
• The main indication of Oral and Maxillofacial Surgery Clinic is the evaluation of the position of the third mandible of the mandible with the lower alveolar nerve.
