Summary:
This literature review is focusing on the research of factors that correlate, predict or they are risk factors for somatoform disorders or somatization in general, including also functional disorders that in the same time are subject of other medical disciplines, other than psychiatry. The papers that have been reviewed were published in PubMed and MEDLINE from 1995 to 2018 based on the existence of any correlation between psychodynamic and psychosocial factors with variable somatization manifestations, as this could be assumed through the review of the abstracts. The goal was in association with the new diagnostic category Somatic Symptom Disorder (SSD) in DSM-5 and some vagueness occurred through the diagnostic criteria of that category, to try to understand the last ones with an integrative way by reviewing all the literature data in a period of about 20 years about them. An important change by the replacement of Somatoform Disorder in DSM-IV with the SSD in DSM-5 was the new diagnostic criterion about the importance of excessive thoughts, feelings and behaviours that correlate with the somatic symptom or symptoms and concerns about one’s health. Identifying psychodynamic and psychosocial predictive factors could help clarifying how to define and measure these thoughts, feelings and behaviours existing on the background of the somatic symptom and the limitations in a person’s functionality associated with it. The review showed multiple possible predictive or positive associated with the somatization factors like mental disorders, dysfunctional cognitions and coping strategies, personality traits, alexithymia, insecure attachment strategies and childhood trauma/neglect/victimization, with some of them appearing repeatedly in studies with different methodologies and on different populations. These predictors could be the subject of future research about the definition of a predictive psychodynamic/psychosocial profile, which could also allow the arrangement of new prevention and treatment strategies, common for all the involved medical disciplines.
