Epidemiology and mechanisms of secondary mesenchymal neoplasms-head and neck sarcomas induced by radiotherapy
Summary:
Radiation-induced sarcomas-RIS sarcomas can occur within 10-12 years (mean latency) and they have a minimum latency period of 3 years.
In addition to the epidemiological data gathered in the present work, potential molecular pathways involved in the development of these invasive mesenchymal neoplasms have been reported. The ATM, ATR and TGF-b molecular pathways play a very important role in repairing the damage that can be caused to the cell by radiation therapy. The major histologic subtypes of RIS reported in the head-neck region are: osteosarcoma, malignant fibrous histiocytoma-MFH, fibrosarcoma, and non-specified sarcoma-NOS which develop mostly in the nasal region and in the sinuses. Radiotherapy-induced sarcomas also occur in the oral cavity as well as in the structures of the temporal bone. The relationship between dose and risk of development RIS has not specified at all but it is widely accepted that doses higher than 55Gy could increase this risk.
Finally, it is commonly accepted that childhood irradiation is one of the major risk factors for the occurrence of such tumors with the risk of developing at peak ages younger than 4 years.
