Summary:
Introduction: Minimally invasive rectal surgery has pushed back the frontiers of organ preservation in rectal cancer patients. Full thickness excision is indicated in selected group of patients.
Purpose: A review of the bibliography concerning the use of transanal endoscopic microsurgery (TEM) / transanal minimally invasive surgery (TAMIS), in T2 adenocarcinoma rectal patients after neo-adjuvant therapy.
Material and method: An extensive search was made in the international literature, using the following keywords in various combinations: "rectal cancer", "neoadjuvant", "transanal endoscopic microsurgery", "transanal minimally invasive surgery". The studies were evaluated for their relevance to the subject, and analyzed to record their results.
Results: According to the counting criteria, a total of 17 studies were eventually identified (15 retrospective or prospective, 2 prospective randomized TEM / TAMIS studies vs radical resection). The local recurrence rates range from 0% - 20% in retrospective and prospective studies, after neoadjuvant chemo-radiotherapy and local resection with minimally invasive methods. On the other hand, in both prospective randomized trials, comparing transanal excision after neoadjuvant chemo-radiotherapy with laparoscopic total mesorectal excision, the rates of local recurrence are similar, with no statistically significant difference in any study.
Conclusions: Until the long-term results of prospective randomized trials are published, the issue of local resection in T2 rectal cancer patients in combination with neoadjuvant chemo-radiotherapy will be a subject of intensive study. Nevertheless, through the presentation of the results of this review, we are encouraged for the possible use of TEM/TAMIS in the future in selected patients.
