Summary:
Background: Combined Anti-Retroviral Therapy (HAART) has led to significant benefits regarding the survival and the quality of life of people living with HIV (PLHIV). Nevertheless, PLHIV suffer from comorbidities and heart diseases, which shorten their life expectancy. Therefore, it is crucial to quantify the years lost due to this and the gap in life expectancy between the general population and PLHIV as well as to identify the factors and conditions which contribute to this.
Methodology: Our data were derived from the multicenter cohort study AMACS, including all patients who were diagnosed in during the period 1996-2018. Initially, a non – parametric approach was implemented by constructing Abridged Life Tables through which I estimated the average number of years left at the ages of 20, 30 and 40 years. The Abridged Life Tables were constructed for the total cohort and for some specific strata as well, such as gender, mode of infection, nadir CD4, level of education completed and period of follow-up. In addition, we implemented a semi-parametric analysis, where mortalities at specific 5-year age-groups were estimated through parametric Survival models.
Results: 7121 20-year old patients were expected to live on average about 49,36 years (47,67 - 51,06). Women compared to men had lower life expectancy. The same was observed for IDUs when compared to man having sex with men and heterosexuals. The gap observed between HIV positive people and the general population is narrowed overtime, but a 10-year gap remained in 2015-2017. Furthermore, life expectancy is increased when the patients follow a healthy lifestyle, such as not being a smoker, not being a drug user and when he pays attention to retaining his health. For the latter a good indicator is the number of CD4 counts at diagnosis.
Conclusion: The gap in the life expectancy between HIV positive people and the general population is reduced over time and the impact of certain conditions and factors could be estimated through appropriate stratifications. s. The increasing over time trend in PLHIV’s mortality rates is observed both, overall and in all studied strata as well, even among IDUs.
