Summary:
Platelet transfusion is a life-saving therapeutic intervention used to treat thrombocytopenia or hemorrhage, while it has become an essential therapeutic mean in modern medicine. The current standard platelet storage method is at room temperature (RT 20-24 ° C) under constant agitation.
Although the clinical benefit of platelet transfusion has been demonstrated, the risk of transmitting bacterial infection is still high as storage at room temperature promotes bacterial contamination. In addition, the short storage time at room temperature (5 days) leads to waste by discarding the
preparations due to their expiration and consequently to a possible lack of availability which is typical today due to the COVID-19 pandemic.
New alternative ways of storing platelets could provide a solution to the above issues, a fact that has led to the conduct of extensive research. In fact, various studies have shown that platelets stored at cold temperatures (1-6o C) show better hemostatic capacity compared to those stored at room
temperature while at the same time there is the possibility of extending storage time. However, despite promising human trials in the 1970s, cold platelet storage was abandoned due to the shorter survival time in the circulation after transfusion.
Cold storage of platelets causes structural, molecular and metabolic changes. These changes, also known as cold platelet storage damage, show some form of activation but can also mean apoptosis. Although cold platelets show elevated apoptotic markers in several studies, the exact mechanism of apoptosis remains unclear. The investigation of various indicators of apoptosis during cold storage has been the subject of many studies that aimed to evaluate the quality and maximum storage time, as
well as to reveal the effects of different storage temperatures. The indicators of aging and apoptosis of stored cold platelets are related to their rapid clearance from the circulation and therefore their investigation could lead to the development of new means and methods of storage and inhibition of apoptotic activity.
