INTRODUCTION: Insomnia is a major public health problem with serious implications for people's quality of life. Chronic insomnia affects 10% of the general population and is associated with increased morbidity and addictive behaviours.
OBJECTIVE: The aim of the study is to describe the epidemiology of insomnia problems and their comorbidity with alcohol use disorders in the general population of Greece. Also, correlations of insomnia with socio-demographic and other variables, with alcohol use disorders, with psychiatric disorders and use of health services are investigated.
METHODS: The study includes a secondary analysis of a 2009-2010 survey on psychiatric morbidity in the general adult population of Greece (4894 participants living in households, response rate 54%). Insomnia problems and psychiatric disorders were assessed with the Revised Clinical Interview Schedule CIS-R and alcohol use disorders with the AUDIT scale.
RESULTS: 11.50% of participants developed clinically significant insomnia and 17.49% overall insomnia problems. 26.11% of participants with clinically significant insomnia had overall alcohol use problems, compared with 21.38% of those without clinically significant insomnia Insomnia was significantly associated with female gender, advanced age, divorce, unemployment, financial difficulties and chronic physical illness. In addition, a mild association between insomnia and overall alcohol use problems was observed, regardless of all studied variables including psychiatric morbidity in the multivariate logistic regression. Finally, there was an increased use of general and mental health services in those who suffered from insomnia compared to healthy participants.
CONCLUSIONS: Insomnia problems are common in the general Greek population and show mild morbidity with alcohol use problems and high with other psychiatric disorders. The observed overuse of health services among adults with insomnia requires the implementation of effective diagnosis and treatment policies at the primary level, with potential secondary benefits in the prevention of alcohol use disorders.
