Summary:
BACKGROUND: JIA is a heterogeneous group of arthritis. Most forms exhibit clinical and genetical similarity with adult-onset arthritis but there are also forms which are restricted to children. For this reason, the nomenclature “JIA” has been developed and suggests the distinction between childhood-onset and adult-onset arthritis. Nevertheless, during the course of time and the progress in medicine, we found restrictions in this approach, necessitating a new approach based on the biological nature of JIA.
AIM: The aim of this review is to present the ILAR classification criteria of JIA, focus on the restrictions of this classification, describe the pathogenesis of JIA and analyze the current data of the new classification based on disease’s biology.
METHODOLOGY: International bibliography was searched on the electronic bases Pubmed and Scopus. Retrospective cohort studies were included and the search was done with the following key words: (“classification”) AND (“juvenile idiopathic arthritis” OR “childhood arthritis” OR “JIA”) AND (“ILAR”) AND (“PRINTO”). The exclusion criteria of articles were the following: (1) title or text irrelevant with our theme, (2) type of study: prospective study, systematic review, case-control study, (3) studies with restricted bibliography and (4) studies out of time reference. Review quality was evaluated using the Preferred Reporting Items for Systematic Review and Meta-analysis (PRISMA). Studies’ selection and quality assessment were conducted by independent researchers.
RESULTS: From the search of international literature, 90 studies emerged. 13 articles excluded because there were duplicated, 10 were excluded because of their incompatible title, 17 of them were excluded after reading their abstract and 7 of them were excluded after reading the full text. After all, 43 articles were included in this review.
CONCLUSIONS: New criteria based on disease’s biology should be applied on the current ILAR classification criteria of JIA, be incorporated in adults’ criteria, in order to include the International Pediatric Rheumatology society. This review aims to present the necessity and current data of the new classification system so that rheumatologists and Pedo- rheumatologists will share a common language. This approach will advance children’s health. The new classification system should be drived by data, biomarkers and the response of patients to therapy so that the remission time will be maximized and the quality of children’s life will be upgraded.
