Summary:
Introduction: Allergy is a non-infectious disease that affects hundreds of people and can cause a wide variety of symptoms, ranging from minor clinical significance to life-threatening. Kounis syndrome refers to an acute coronary episode, induced in the context of an allergic reaction. Although the association between coronary syndromes and allergic reactions is widely known, the significance of the increase in cardiac markers in the context of allergic reactions, such as cardiac troponin, remains understudied.
Aim: The aim of this study was to assess high-sensitivity troponin levels in patients presenting to the emergency department with an allergic reaction.
Material and Methods: The study sample consisted of 200 patients who came to the emergency department of Patras General Hospital, due to a type I allergic reaction. The patients underwent continuous electrocardiograms and high sensitivity troponin measurement (hs-cTn).
Results: Of the 200 patients in the study, 52.5% were male. 5.0% had diabetes mellitus and 24.0% had arterial hypertension. 73.5% of the cases were diagnosed with urticaria. Myocardial damage was detected in a total of 24 patients (12.0%), with 6.0% diagnosed with Kounis syndrome and 6.0% with myocardial damage without an ischemic event. A statistically significant difference was detected between the populations that presented myocardial damage compared to those that did not in terms of risk factors diabetes mellitus (p<0.001), dyslipidemia (p=0.029), arterial hypertension (p=0.023) and active smokers (p=0.017).
Conclusion: The incidence of myocardial injury in the total study sample was 12.0%, and Kounis syndrome was observed in 6.0% of patients. Patients with myocardial damage were more likely to be smokers and to have a personal history of arterial hypertension, diabetes mellitus and dyslipidemia.
