Summary:
Introduction: Heart transplantation is the treatment of choice for end-stage heart failure.
After transplantation, lifelong immunosuppressive treatment is required to protect the graft. However, it has been observed that adherence to immunosuppressive treatment is a challenge for patients, which demonstrates the importance of investigating the phenomenon of non-adherence.
Objectives: To investigate adherence to immunosuppressive treatment of patients after
heart transplantation through a systematic review of international literature.
Material and Methods: A systematic review was performed using two databases,
"PubMed" and "Scopus". The included studies were primary, quantitative, published in
English and within the time period 2000-2024. The keywords used for the search were
based on the three main pillars of the research question: “heart transplantation”,
“medication adherence” and “immunosuppressive treatment”..
Results: After searching the literature in the databases, a total of 234 articles were found,
of which 18 were included in the systematic review, based on the inclusion criteria set. The mean age of the participants was between 46-61 years and the majority were male. In addition, the majority of studies used self-report methods to assess adherence to immunosuppressive treatment and only 5 studies used more than one assessment method. Both the degree of adherence and the classification of adherence varied widely in the included studies. Overall adherence was found in 7 studies 24.1% - 46.1% , and in the majority of studies above 50%, with 2 studies reporting a rate above 80%. The dimensions of adherence investigated in the majority of studies were divided into "drug holidays", “taking nonadherence” and “timing nonadherence” to the immunosuppressive treatment , while in the majority the lowest adherence rate was noted in the time of intake.
Conclusions: Adherence to immunosuppressive treatment in patients after heart
transplantation is not sufficient. Further research is needed, using reliable methods to
assess adherence, to investigate the factors leading to non-adherence and to implement
interventions to enhance adherence.
