Summary:
Introduction:Obesity has been evolved as an epidemic and therapy remains an unsolved global problem. Pharmacolocical and surgical approaches may be used for the treatment of obesity.
Aim: The aim of this literature review is to access the efficacy of pharmacological versus surgical treatment of obesity, using weight loss and metabolic parameters improvement as outcomes.
Method: We conducted a literature search in databases Pubmed, Google Scholar, Medline, Embase και Cochrane, using the following key-words: «bariatric surgery», «GLP-1 – GIP», «tirzepatide», «GLP-1», «weight loss», «BMI» και «metabolic parameters». We included studies with patients aged > 18 years old, with Body Mass Index (ΒΜΙ) > 25 kg/m2, haven ungergone metabolic surgery or were prescribed GLP-1 receptor agonists or GLP1/GIP. We excluded studies in the population under 18 years old, in animals and in people with normal BMI.
Results: Surgical treatment is more effective than treatment with GLP-1 receptor agonists in reducing body weight and improving metabolic parameters. Tirzepatide, a double GLP1/GIP agonist and high doses of semaglutide have proven to be effective in reducing body weight and improve metabolic parameters. Up to date there is no study comparing tirzepatide or semaglutide to bariatric surgery concerning efficacy.
Conclusion: Large, prospective studies are needed to compare head-to-head tirzepatide and semaglutide to surgical intervention.
