<dc:description xml:lang="en">Thyroid disorders during pregnancy constitute a major public health issue, as they affect both maternal and fetal health. This study is a systematic literature review aiming to investigate the prevalence, risk factors, maternal and neonatal outcomes, as well as the contemporary diagnostic and therapeutic approaches related to thyroid disorders during pregnancy. The review demonstrated that subclinical hypothyroidism is the most common thyroid disorder in pregnancy, with a worldwide prevalence ranging from 2% to 10%, while autoimmune thyroid diseases, predominantly Hashimoto’s thyroiditis, affect a substantial proportion of pregnant women. The presence of thyroid autoantibodies is associated with an increased risk of miscarriage, preterm delivery, and postpartum thyroiditis. Furthermore, thyroid dysfunction has been linked to adverse obstetric outcomes and to neurodevelopmental effects in the offspring. Therapeutic management is mainly based on levothyroxine administration for hypothyroidism and antithyroid drugs for hyperthyroidism, with an emphasis on individualized dose adjustment and systematic monitoring. Emerging research directions focusing on the role of the gut microbiome and complementary interventions are also of particular interest. In conclusion, early diagnosis, close monitoring, and an individualized therapeutic approach are critical factors for improving maternal and fetal outcomes. The present review highlights the need for further targeted research and for the continuous updating of clinical guidelines.</dc:description>
