<dc:description xml:lang="en">Introduction: The increasing frequency of mass casualty incidents highlights the need for operational preparedness. Hemorrhage is the leading cause of potentially preventable death and early resuscitation is the reason that places transfusion in the prehospital setting. Methods: An English language literature review (2014-2024) was conducted, through PubMed, Scopus, Cochrane και Google Scholar databases. Thirty-five articles were included according to PRISMA-ScR guidelines. Results: Prehospital transfusion has been mainly studied in military settings, with variety in blood products usage over time, depending on prehospital barriers. Early transfusion in the field and en route is associated with reduced mortality. Response programs have begun developing and, occasionally, activated in civilian settings. Discussion: The experience from military mass casualty incidents supports the presence of transfusion in prehospital setting, though further targeted research, documentation, and procedures’ adaptation is required. Conclusion: Integrating prehospital transfusion in mass casualty events requires systematic planning, clear protocols, training, and authority- community collaboration. Limitations: The retrospective and heterogenous nature of the studies, as well as the predominance of military data limit generalization to civilian mass casualty events.</dc:description>
