<dc:description xml:lang="en">The digitization and decentralization of clinical trials (Decentralized Clinical Trials – DCTs) are redefining how clinical research is designed and conducted. The COVID-19 pandemic accelerated the adoption of innovative data collection and monitoring methods, including telemedicine, wearable devices, and real world evidence. Regulatory authorities responded by issuing guidelines to facilitate the integration of DCTs, while emerging technologies such as artificial intelligence and blockchain are being deployed to ensure compliance, accuracy, and traceability. However, this progress must not overlook the persistent inequalities in trial design and accessibility. The underrepresentation of vulnerable populations—such as women, the elderly, pregnant individuals, and ethnic minorities—necessitates a critical revision of inclusion criteria. Strengthening bioethics, producing clear and understandable information for patients, and enhancing participatory mechanisms are essential steps toward the social legitimacy of research. At the same time, globalization demands harmonization of regulatory frameworks and capacity-building support for countries with limited infrastructure. Scientific rigor, data governance, and the involvement of smaller research sponsors remain vital components of an inclusive and trustworthy research environment. The ongoing challenge lies in balancing innovation with justice, ensuring that clinical trials are not only effective but also ethically sound and socially responsible.</dc:description>
