<dc:description xml:lang="en">Objective: The aim of this retrospective study is to record, analyze, and evaluate the data concerning patients who underwent total thyroidectomy, focusing both on the main indications that led to surgical decision-making and on the postoperative complications and oncological outcomes observed. Materials and Methods: A retrospective study was conducted on 312 patients who underwent total thyroidectomy at the 1st Surgical Department of Sismanogleio General Hospital during the period 2019–2024. The study examined the surgical indications, histopathological findings, postoperative complications, and clinical outcomes of the patients. Results: A total of 71.1% of the patients were women, with a mean age of 54.3 years. Benign conditions accounted for 80.1% of preoperative indications, while papillary carcinoma was the most common malignancy (17.3%). Histopathological analysis showed that approximately 45% of the cohort had papillary carcinoma, while among benign diseases, nodular hyperplasia (27.6%) and thyroiditis (autoimmune/lymphocytic, ~18.9%) predominated. The most frequent postoperative complication was transient hypocalcemia (25.3%), whereas permanent hypocalcemia occurred in only 1.9% of cases. Additionally, more than 20% of the surgical specimens revealed an incidental carcinoma, mainly micropapillary, highlighting the importance of total thyroidectomy even for seemingly benign conditions. No mortality was recorded. Conclusions: Total thyroidectomy is a safe and effective procedure with low rates of permanent complications when performed by specialized surgical teams. Careful identification of the parathyroid glands and the use of modern hemostatic and visualization techniques significantly reduce morbidity and improve patient outcomes.</dc:description>
